• Locate your U‑Card—you’ll see the Card Number on the front.
  
  
• Provide your Member ID or other requested data.
  
  
  

3. Verify your identity

• Date of birth
  
  
• Last four digits of your SSN
  
  
• Any other info UHC requests
  
  
  

4. Sign in or create an account

• If you already have a UHC account, simply log in.
  
  
• If not, select “Create Account” and follow on-screen instructions:
  
  
  • Enter your email
    
    
  • Pick a username & password
    
    
  • Set up security questions
    
    
    

5. Submit and confirm

• Review all your information to ensure it’s correct.
  
  
• Hit “Submit” or “Activate.”
  
  
• You should see a confirmation message on the screen once activation is successful.
  
  

6. (Optional) Use the UHC app

• Download the UnitedHealthcare mobile app on iOS or Android for easy access to your digital card, benefits, and claims.
