We need all your personal information to process a withdrawal. You can enter this info on your "Account Settings" page.   DO NOT USE AUTO-FILL.  Please make sure you're typing everything in manually 1 by 1.  Hit the "Save Changes" button after making updates. 

Before your first withdrawal, you'll also be asked to submit a picture of a valid, government-issued photo ID proving you're 18.
